What to Expect
The Initial Evaluation will involve a one-on-one session lasting one hour. This will include a "whole body" kinetic chain assessment of strength, flexibility, range of motion, and functional performance. The initial visit will also include any manual techniques indicated by evaluation findings and instruction in an individualized home exercise program.
Follow up visits will consist of a reassessment of initial findings, manual techniques as needed, and fine-tuning and progression of home exercise program in order to progress you as quickly as possible to your life/fitness goals.
How to Get Started
Physical Therapy requires a prescription from your physician. You can contact your physician and have them give you a prescription that your bring to your Initial Evaluation, or have them fax the prescription to: 877-887-7721. Also, go to the "Important Forms" page of this website to print and fill out important forms to bring to your initial visit.
Insurance
I am NOT contracted as an in-network provider for any third-party payers. I can, however, provide you with the appropriate paperwork to submit to your insurance company if you would like to request reimbursement.
I am NOT a Medicare Provider and according to Federal law, am not able to provide Physical Therapy to Medicare Beneficiaries, even if you have exceeded your Part B benefits and would be paying out-of-pocket.
